I read some things on Reddit that speculates that it could be a controller problem (audio issues), and so I switched controllers to confirm but that doesn’t seem to be the case. There were also some that said that I need to do a hard reset, in which I did but that hadn’t worked either. Others said that I needed to rebuild the database, and so I did and to no avail. I also switched HDMI cords and whatnot, NOTHING! I’ve ruled out most things which leaves one thing left: the disc reader. Admittedly, I did not clean out my PS4 slim since I bought it 4 years ago. I don’t know if it’s build up of dust and dirt, or if the disc reader is compromised. Please help! I don’t have warranty on it anymore.
