PS5 Slim EDM-041 Weird Issue: Safe Mode works fine, currently in a "corrupt database" bootloop

Originally the console would only show red blocks on the screen + no safe mode. I was worried it might be APU but I replaced the encoder IC and HDMI port just in case. After that I could boot into safe mode with no artifacting, but still got red blocks when trying to boot normally.

I tried reinstalling the system firmware, repairing console storage, clearing cache, and rebuilding database (All of these options would say they succeeded btw). When rebuilding database the output would switch to 1080p with the progress bar, and there would be no screen artifacts at all, so it seems like it may not be encoder related.

Since these all showed they succeeded I was pretty hopeful, but now it boots to a screen saying the database is corrupt and needs repaired, but before I can select to continue half the screen shows white dots and then the display will go blank and reset. During that process the console will not actually shutdown or restart, the white light stays on the entire time and the controller will connect to the console. I’m just seemingly unable to select the rebuild database option. Sometimes when this happens I’ll try to force shutdown via the power button and it will beep 3 times with the white light staying on, and I’ll have to unplug it to get it to turn off. Due to that symptom, I replaced the BIOS IC but it’s having the exact same issues.

I’ve scoured the forum and can’t find any similar problems.
Some more misc things I’ve tested:

  • HDMI encoder diode readings and voltages (power and standby are present)
  • All fuses good, all other standby and power voltages good.
  • All HDMI port diode readings are correct when off but all turn to 000 when turned on (I don’t know if that’s normal but it will display just fine even when they all read 000)
  • The mosfet above F7002 seems to get warm under thermal camera but that seems normal.
  • No UART error codes at all.

I’m thinking probably SSD controller but the symptoms don’t seem like they really match considering I was able to successfully reset system firmware and rebuild database when in safe mode. Any help is very appreciated!
